Hi Margo, thank you for sharing these issues with us and I wish that you had called into our office at (866) 866-1999 to go over your concerns as this is the first time that I'm learning about these problems. I do see what you are referring to about the beneficiaries and this kind of issue is something that I or any of my associates in the office would assist you to have corrected by the insurance company and they can send you a confirmation letter to show their records have been updated. As far as tax withholding, many insurance companies utilize the IRS Form W-4P and it is not very clear how to complete the form to request a specific amount to be withheld. Many of my clients either opt for No Withholding (and handle the taxes quarterly as you mentioned) or allow the insurance company to administer the "default withholding" by leaving the W-4P form blank. Either way, I'm sorry to hear your disappointment. Please do not hesitate to call our office in case you are still experiencing any issues with your contract.
